I have only seen this on my sole instrument and not tested with any others, but when I first got mine, I used it once and the uses went from 400 to 399. Now, I've used my instrument many times and it remains at 399 uses remaining.

I've used multiple instruments and not run into this issue. Try moving it in your pack; the display may not be updating.

I just tried a few times and its still the same.

Does it make a difference if it was a purchase from an NPC versus a player made one?

Not sure. Mine have all been NPC purchases

Just double clicking it. I have Music and Discord.

If you use Discord it may take the uses down. I have gone through many player-crafted and NPC bought ones. Maybe just double-clicking it isn't considered a use?

I tried it with Discord and that did the trick - its based on skill usage, not the double clicking.
